Özet

AMAÇ: Aksilla tutulumu olan meme kanseri hastalarında neoadjuvan kemoterapi (NAKT) sonrası frozen kesit analizi çoğu zaman kullanılmaktadır. Ancak tekniğin ameliyata ekstra maliyet katması, ameliyat süresini uzatması, frozen incelemede birkaç lenf nodunda şübheli pozitiflik saptanması durumunda gereksiz aksiller diseksiyona (ALND) gidilmesi, bunun sonucunda lenfödem, kronik ağrı, ekstremite fonksiyon kaybı gibi diseksiyona bağlı morbidite olasılığının artması gibi sebeplerden dolayı frozen kesit analizinin rutin kullanılması tartışmalı bir konudur. Literatürde, diseksiyon yapılmadan aksillanın durumu sentinel lenf nodu biyopsisi (SLNB) ile değerlendirilen, neoadjuvan sonrası minimal rezidüel hastalığı olanlarda ise ameliyat sonrası aksillaya radyoterapi uygulanan hastalarda uzun dönemde diseksiyon yapılanlarla kıyasta anlamlı fark olmadığını gösteren çalışmalar mevcuttur. Çalışmamız, neoadjuvan tedavi sonrası aksiller tam yanıt alınan meme kanser hastalarında peroperatif sentinel lenf nodu frozen yapılmasının cerrahi sonuçlar, nüks oranları ve sağkalım üzerine etkisi olup olmadığı araştırmıştır. GEREÇ YÖNTEM: 1 ocak 2018 – 31 aralık 2023 yılları arasında lokal ileri meme kanseri tanısı ile neoadjuvan tedaviye yönlendirilen ve aksiller yanıtı tam olup ameliyat edilen hastaların kayıtları retrospektif olarak incelendi. Frozen kesit inceleme yapılan ve yapılmayan olmak üzere 2 gruba ayrıldı. Hastaların demografik, klinik ve patolojik özellikleri, cerrahi sonuçlar ve sağkalım verileri analiz edildi. İstatistiksel analizlerde Bağımsız örneklem t testi, Ki-kare testleri, Kaplan-Meier ve Cox regresyon modelleri kullanıldı. BULGULAR: Çalışmanın kriterlerine uygun toplam 154 hasta saptandı. Hastaların yaş ortalaması 49.6±11 yıl idi. Hastaların %69,5'ine peroperatif frozen incelemesi uygulanırken, %30,5'ine uygulanmadığı saptandı. Gruplar arasında yaş, tümör tipi ve evre dağılımı açısından anlamlı fark bulunmadı. Moleküler alt gruplar ile frozen işlemi yapılma durumu arasında istatistiksel olarak anlamlı bir fark saptanmadı (p=0,113). Hormon reseptörü (HR) (+) ve Her2 (-) grubu en yaygın alt grup olup frozen yapılanların %50,5'ini, yapılmayanların ise %31,9'unu oluşturmaktaydı. Peroperatif frozen uygulanan grupta 16 hastaya (%15) sentinel lenf nodu biyopsisi ile birlikte aksiller lenf nodu diseksiyonu yapılırken, 8'inin diseksiyon materyalinde metastatik lenf nodu izlenmedi. Her iki grupta birer hasta olmak üzere toplam 2 (%1,3) hastaya bir sonraki seansta ALND yapıldı. Nihai patolojik incelemede, frozen yapılan grupta %43, yapılmayan grupta ise %34 oranında rezidü metastatik lenf nodu (ypN+) saptandı. Çıkarılan lenf nodu sayıları (SLN: 3,51±1,66 ve 3,49±1,93; non-SLN: 2,49±1,38 ve 2,45±1,70) ve pozitif lenf nodu sayıları (1,64±0,87 ve 1,93±0,92) açısından da gruplar arasında anlamlı fark yoktu. Otuz sekiz aylık medyan takipte lokal nüks %2,6 (n=4), uzak metastaz %8,4 (n=13) ve kansere bağlı ölüm %3,9 (n=6) oranında görüldü. Tüm lokal nüksler, ölümler ve lenf ödem vakaları (n=3) yalnızca frozen yapılan grupta idi, ancak istatistiksel anlamlılık yoktu. Frozen yapılmayan grupta daha düşük metastaz oranı (%2,1 ve %11,2; p=0,111) gözlendi, ancak bu grupta takip süresi daha kısaydı (50 ay karşı 29 ay). SONUÇ: Peroperatif frozen incelemesinin aksiller cerrahi sonuçları, nüks oranları ve sağkalım üzerinde anlamlı bir etkisi yoktu. Bulgular frozen yapılmamasının nüks ve komplikasyon riskini artırmadığını düşündürmekle birlikte, kesin sonuçlar için daha uzun takipli çalışmalar gereklidir.

AIM: In breast cancer patients with axillary involvement, intraoperative frozen section analysis is frequently utilized following neoadjuvant therapy (NAC). However, the routine use of this technique remains controversial due to several reasons. İt adds extra cost to the surgical procedure, prolongs operative time, and may lead to unnecessary axillary dissection (ALND) when suspicious positivity is detected in a few lymph nodes during frozen examination. Consequently, the risk of dissection-related morbidities such as lymphedema, chronic pain, and upper extremity functional impairment increases.In the literature, studies have shown that in patients whose axillary status was assessed by sentinel lymph node biopsy (SLNB) without axillary dissection, and in those with minimal residual disease after neoadjuvant therapy who subsequently received axillary radiotherapy, there was no significant difference in long-term outcomes compared with patients who underwent axillary dissection. The present study aimed to investigate whether intraoperative sentinel lymph node frozen section analysis in breast cancer patients who achieved a complete axillary response after neoadjuvant therapy had any impact on surgical outcomes, recurrence rates, and overall survival. MATERIALS AND METHODS: A total of 154 female patients who were diagnosed with locally advanced breast cancer, received neoadjuvant therapy, achieved complete axillary response, and subsequently underwent surgery between 1 january 2018 and 31 december 2023 were retrospectively reviewed. Demographic, clinical, and pathological features, surgical outcomes, and survival data were analyzed. Statistical analyses were performed using independent samples t-tests, chi-square tests, Kaplan-Meier survival analysis, and Cox regression models. RESULTS: The study included 154 women with a mean age of 49.6 ± 11 years. Intraoperative frozen section was performed in 69.5% of patients, while 30.5% did not undergo frozen analysis. There were no significant differences between the groups in terms of age, tumor type or stage. No statistically significant association was found between molecular subtypes and the use of frozen section (p = 0.113). The most common molecular subtype was HR(+)/HER2(-), comprising 50.5% of the frozen group and 31.9% of the non-frozen group. Among the patients who underwent frozen section, 16 (15%) underwent both sentinel lymph node biopsy and axillary lymph node dissection; 8 of these had negative nodes in the dissection specimen. A total of two patients (1.3%) one from each group underwent delayed ALND in a second surgical session. Final pathology revealed residual metastatic lymph nodes (ypN+) in 43% of the frozen group and 34% of the non-frozen group. There were no significant differences between the groups in terms of the number of lymph nodes removed (SLNs: 3.51 ± 1.66 vs. 3.49 ± 1.93; non-SLNs: 2.49 ± 1.38 vs. 2.45 ± 1.70) or the number of positive nodes (1.64 ± 0.87 vs. 1.93 ± 0.92). After a median follow-up of 38 months, the rates of local recurrence, distant metastasis, and cancer-related death were 2.6% (n=4), 8.4% (n=13), and 3.9% (n=6), respectively. All local recurrences, deaths, and cases of lymphedema (n=3) occurred in the frozen group, though the differences were not statistically significant. A lower rate of metastasis was observed in the non-frozen group (2.1% vs. 11.2%; p = 0.111), although this group had a shorter follow-up period (29 vs. 50 months). CONCLUSIONS: Intraoperative frozen section analysis of the axilla did not significantly affect surgical outcomes, recurrence rates, or survival. While the findings suggest that omitting frozen section does not increase the risk of recurrence or complications, studies with longer follow-up are needed to confirm these results.
