Özet

Maternal hipotiroidizm, annenin tiroid bezinin yeterince hormon üretememesi sonucu gelişen bir durumdur ve plasentanın gelişimini olumsuz etkileyebilir. Hipotiroidizm, plasentadaki antiinflamatuar ortamı bozarak preeklampsi, intrauterin büyüme geriliği, spontan abortus, kardiyak disfonksiyon, nöral gelişim bozuklukları, anemi, plasental ablasyon ve düşük doğum ağırlığı gibi komplikasyonlara neden olabilir. Melatonin, uyku düzenini ve biyolojik ritmi düzenleyen bir hormon olup, aynı zamanda plasentada da sentezlenir. Melatonin, oksidatif stresi azaltarak antioksidan etkiler gösterir, immün yanıtları düzenler ve fetüsün nörolojik gelişimi ile uyku-uyanıklık döngüsünü destekler. Bu nedenle gebelikte fetüs ve anne arasındaki biyolojik dengeyi korumada önemli rol oynar. Fetomaternal immün tolerans, yarı allojenik fetüsün anne tarafından reddedilmeden kabulünü sağlar. Bu mekanizmada FoxP3+ regülatör T hücreleri (Treg) merkezi bir rol oynar. FoxP3+ Treg hücreleri, aşırı bağışıklık yanıtlarının engellenmesi ve otoimmün hastalıkların kontrol altına alınmasında görevli özel bir T hücre alt grubudur. FoxP3 ifadesi, bu hücrelerin gelişimi ve işlevi için gereklidir. Pek çok gebelik patolojisinde bu hücre popülasyonunun azaldığı gösterilmiştir. Bu çalışmada, hipotiroidili ve sağlıklı sıçanların plasentalarında FoxP3+ hücrelerin dağılımını ve melatoninin bu dağılım üzerindeki etkisini araştırdık. Dört grup oluşturuldu: kontrol, hipotiroidi (%0,05 PTU), melatonin (10 mg/kg), hipotiroidi + melatonin. Uygulamalar içme suyuyla yapıldı. Sıçanlar gebe bırakıldı ve gebeliğin 19. gününde sezaryenle plasentalar çıkarıldı. Dokuda FoxP3+ hücrelerin belirlenmesi icin FoxP3 ile ve trofoblast değişiminin belirlenmesi için CK7 ile immunohistokimya (IHC) yapıldı. Ayrıca H&E boyaması ile gruplar arasında plasentadaki histomorfolojik değişiklikler değerlendirildi. Histokimyasal olarak hipotiroidi grubunda kalınlaşan bazal zonun melatonin ile birlikte verildiğinde azaldığını gözlemledik. Glikojen hücre gruplarının hipotiroidi grubunda artış gösterdiğini gözlemledik. Vaskülarite oranları değerlendirildiğinde melatonin ve hipotiroidi+melatonin gruplarında vaskülaritenin arttığını değerlendirdik. İHC sonucuna göre hipotiroidi grubunda FoxP3 azalma eğilimi göstermiş ve melatonin grubunda artış göstermiştir. Hipotiroidi ve melatonin arasında anlamlı fark bulunmuştur. CK7 değerlendirildiğinde hipotiroidi grubunda azalmış ancak anlamlı bulunmamıştır. Sonuç olarak plasentada FoxP3+ hucrelerin maternal hipotiroidide azaldığı ve melatonin ile arttığı, melatoninin hipotiroidi ile bozulan feto-maternal immun toleransı yeniden düzenleyebileceği sonucuna ulaştık. Bu bulgular ile hipotiroidi ve melatoninin plasental Foxp3+ üzerinde birlikte yaptığı etkiyi gösteren ilk çalışma bizim çalışmamızdır. Melatoninin hipotiroidik gebelerde etkilerinin ve kullanımının ayrıca FoxP3'ün fetomaternal immuntoleranstaki etkilerinin daha fazla çalışma ile araştırılmasına ihtiyaç vardır.

Maternal hypothyroidism is a condition characterized by insufficient thyroid hormone production in the mother and can adversely affect placental development. Hypothyroidism may disrupt the anti-inflammatory environment of the placenta, contributing to complications such as preeclampsia, intrauterine growth restriction, spontaneous abortion, cardiac dysfunction, impaired neural development, anemia, placental abruption, and low birth weight. Melatonin is a hormone primarily known for regulating sleep and circadian rhythms, and it is also synthesized in the placenta. Melatonin exhibits antioxidant effects by reducing oxidative stress, modulates immune responses, and supports fetal neurological development and circadian rhythm regulation. Therefore, it plays a crucial role in maintaining the biological balance between mother and fetus during pregnancy. Fetomaternal immune tolerance ensures the acceptance of the semi-allogeneic fetus by the maternal immune system without rejection. In this mechanism, FoxP3+ regulatory T (Treg) cells play a central role. FoxP3+ Treg cells are a specialized T cell subset responsible for suppressing excessive immune responses and maintaining immune homeostasis to prevent autoimmunity. FoxP3 expression is essential for the development and function of these cells. A reduction in Treg cell populations has been reported in various pregnancy-related pathologies. In this study, we investigated the distribution of FoxP3+ cells in the placentas of hypothyroid and healthy rats and evaluated the effect of melatonin on this distribution. Four groups were established: control, hypothyroid (0.05% PTU), melatonin (10 mg/kg), and hypothyroid + melatonin. All treatments were administered via drinking water. Rats were mated, and placentas were collected via cesarean section on gestational day 19. To detect FoxP3+ cells in the tissue, immunohistochemistry (IHC) was performed using antibodies against FoxP3. To evaluate trophoblast differentiation, IHC with cytokeratin-7 (CK7) was also conducted. Additionally, histomorphological changes in the placenta across the groups were assessed using hematoxylin and eosin (H&E) staining. Histochemically, an increased thickness of the basal zone observed in the hypothyroid group was reduced with melatonin administration. Glycogen cell clusters were increased in the hypothyroid group. Vascularity assessments indicated increased vascularization in both the melatonin and hypothyroid + melatonin groups. According to IHC analysis, FoxP3 expression showed a decreasing trend in the hypothyroid group and an increase in the melatonin group. A significant difference was found between the hypothyroid and melatonin groups. CK7 expression was reduced in the hypothyroid group, although the difference was not statistically significant. In conclusion, FoxP3+ cells were found to be decreased in the placenta under maternal hypothyroidism and increased with melatonin administration. These findings suggest that melatonin may help restore fetomaternal immune tolerance disrupted by hypothyroidism. To our knowledge, this is the first study to demonstrate the combined effect of hypothyroidism and melatonin on placental FoxP3+ cells. Further studies are needed to explore the potential use of melatonin in hypothyroid pregnancies and the role of FoxP3 in fetomaternal immune tolerance.
