Özet
SP'li çocuğun rehabilitasyonunda adaptif malzeme gerekli ve yararlı bir yardımcıdır. Ülkemizdeki SP'li çocukların adaptif cihaz kullanımı hakkında bilgi edinmek amacıyla; tanımlayıcı bir çalışma olarak, İstanbul ilinde Özel Eğitim ve Rehabilitasyon Merkezlerinde fizyoterapi programına alınmakta olan SP'li çocuklarda adaptif cihaz kullanımı motor fonksiyon düzeyleriyle ilişkili olarak değerlendirilmiştir. Bu amaçla Eylül 2014 ile Haziran 2015 tarihleri arasında beş merkezde 0-18 yaş arası 93 SP'li çocuk ile aileleri ve pediatrik rehabilitasyonda çalışan 50 fizyoterapist çalışmaya dahil edilerek, Serebral Palsili Çocuk Adaptif Cihaz Değerlendirme Formu ve Fizyoterapistler İçin Adaptif Cihaz Değerlendirme Formu doldurulmuş, Kaba Motor Fonksiyon Sınıflama Sistemi (KMFSS) skorları, kullanılan adaptif cihazlar araştırılmıştır. 93 olgudan 28'i adaptif cihaz kullanmaktaydı. KMFSS seviyelerine göre öngörülen adaptif cihaz kullanım gerekliliği %67,7 gerçekte kullananların oranı ise % 30,1 olarak bulunmuştur. Ailelerin çoğu adaptif cihazları temin ederken başta ekonomik zorluk (%81) olmak üzere SGK işlemlerinde (%14,2) ve öneri/bilgi düzeyinde (%4,8) zorluklar ile karşılaştığını belirtmiştir. Ailelerin çoğunluğu adaptif cihaz kullanımının rehabilitasyon sürecinden duydukları memnuniyeti orta ve ortanın biraz üzeri seviyede etkilediğini belirtmiştir. Fizyoterapistlerin büyük çoğunluğu SP'li çocukların rehabilitasyonunda adaptif cihaz kullanmanın önemli bir yeri olduğunu ve fizyoterapiye ortanın biraz üstünde etkisi olduğunu belirtmişlerdir. Sonuç olarak SP'li çocuklarda adaptif cihaz kullanımının KMFSS seviyelerine göre öngörülen kullanım düzeyinin altında (%55,6) yer aldığı ve ailelerin adaptif cihazların temininde zorluklar ile karşılaştığı, fizyoterapistlerin SP rehabilitasyonunda adaptif cihaz kullanımına önemli bir yer ayırdığı görülmüştür.

Assessment of the Use of Adaptive Devices in Childeren with Cerebral Palsy Adaptive devices are necessary and helpful materials in the rehabilitation of the child with Cerebral Palsy (CP). The aim of this study was to obtain information about the use of adaptive devices in childeren with CP in Turkey. Accordingly a descriptive study was planned and conducted on children with Cerebral Palsy who received physiotherapy and education in Special Education and Rehabilitation Centers in Istanbul. The existence and use of adaptive devices in relation to the level of motor function was evaluated. Between September 2014 and June 2015, 93 children with CP aged 0-18 from five centers with their families and fifty physiotherapists, working in pediatric rehabilitation were included to this study. The Children with Cerebral Palsy Adaptive Devices Evaluation Form and The Adaptive Devices Evaluation Form for Physiotherapists were filled, Gross Motor Function Classification System (GMFCS) scores and used adaptive devices types were investigated. 28 of 93 children with CP used adaptive devices. Requirements of the use of adaptive devices according to the GMFCS levels was found to be 67,7%. The actually use of Adaptive Devices in Childeren with Cerebral Palsy was found to be 30,1%. Most of families reported that they encountered to mainly economic challenges (81%), the process SGK (14,2%) and recommendations/information at the level of difficulty (4,8%) while adaptive equipment providing. The majority of parents stated that use of adaptive devices have moderate positive impact on the level of their satisfaction with the rehabilitation process. The vast majority of physiotherapists stated that use adaptive devices in children's with CP rehabilitation plays an important role and it's effect was estimated middle and slightly above in the physiotherapy. In summary, level of the use of adaptive devices in children with CP showed where below the level (55,6%) of use proposed by the GMFCS. The families faced difficulties in the supply of adaptive devices. The physiotherapists an important place to the use of adaptive devices in the CP rehabilitation.
