Özet
Bu çalışmada C.spinosa + glutamin kombinasyonunun diyabetik yaralarda potansiyel etkisinin araştırılması amaçlanmıştır. Streptozotosin (STZ) ile diyabet oluşturulan 42 adet yetişkin erkek Wistar-albino sıçan yedi gruba ayrılmıştır: kontrol, tedavi edilmeyen, 0,2 g/kg glutaminle tedavi edilen, topikal ve oral gavaj yoluyla 20 mg/kg C.spinosa ile tedavi edilen ve topikal ve oral gavaj yoluyla 20 mg/kg C.spinosa + 0,2 g/kg glutamin kombinasyonuyla tedavi edilen gruplar olarak planlanmıştır. Deney hayvanları yara iyileşmesinin 7. gününde feda edilmiştir ve yara dokularında glutatyon (GSH), malondialdehit (MDA), nitrik oksit (NOx), MMP-2, MMP-9, kolajen, protein karbonil (PC), askorbik asit (AA), seviyeleri spektrofotometrik metotla ölçülmüştür. Yara doku morfolojik ve histolojik olarak incelenmiştir. MDA seviyeleri tüm tedavi edilen gruplarda, tedavi edilmeyen gruba göre anlamlı şekilde düşmüştür (p<0,05). GSH seviyeleri tüm tedavi edilen gruplarda, tedavi edilmeyen gruba göre anlamlı şekilde yükselmiştir (p<0,05). Tüm tedavi edilen grupların NOx seviyeleri, tedavi edilmeyen gruba göre istatistiksel olarak azalmıştır (p<0,05). Tüm tedavi edilen grupların yara dokusu MMP-2 ve MMP-9 düzeyleri, tedavi edilmeyen gruba göre istatistiksel olarak azalmıştır (p<0,05). Tüm tedavi edilen grupların yara dokusu kolajen düzeyleri, tedavi edilmeyen gruba göre istatistiksel olarak artmıştır (p<0,05). Tedavi edilen grupların PC seviyeleri, tedavisiz gruba kıyasla istatistiksel olarak azalmıştır (p<0.05). AA seviyeleri tüm tedavi edilen gruplarda, tedavi edilmeyen gruba göre istatistiksel olarak anlamlı şekilde yükselmiştir (p<0.05). Morfolojik incelemelerde tüm tedavili grupların tedavisiz gruba göre yara kapanma oranları daha yüksek gözlemlenmiştir (p<0,001). Histolojik değerlendirmeler sonucunda tüm tedavili gruplarda yeniden epitelizasyon, normale yakın bir epidermal kalınlık, keratinizasyonun varlığı, granülasyon dokusunun ve yeniden şekillenmenin normale yakın olması gözlemlenmiştir. İnflamasyon ve nekroz tüm tedavili gruplarda tedavisiz gruba kıyasla daha hafif gözlemlenmiştir. C.spinosa ve glutamin kombinasyonu, yara iyileşmesinde oksidatif stresi azaltıcı bir etki göstermiştir. C.spinosa ve glutamin kombinasyonu, diyabetik yara modelinde yara iyileşmesini olumlu yönde etkilemiştir. Bu çalışma sonucunda C.spinosa ve glutamin bileşenlerinin birlikte ya da ayrı ayrı kullanılmasının oksidatif hasarı azaltmak ve yara onarımını artırmak için umut verici bir strateji olabileceğini düşündürmektedir.

The aim of this study was to investigate the potential effect of C. spinosa + glutamine combination on diabetic wounds. Forty-two adult male Wistar-albino rats with streptozotocin (STZ)-induced diabetes were divided into seven groups: control, untreated, treated with 0.2 g/kg glutamine, treated with 20 mg/kg C.spinosa by topical and oral gavage, and treated with a combination of 20 mg/kg C.spinosa + 0.2 g/kg glutamine by topical and oral gavage. The experimental animals were sacrificed on the 7th day of wound healing and glutathione (GSH), malondialdehyde (MDA), nitric oxide (NOx), MMP-2, MMP-9, collagen, protein carbonyl (PC), ascorbic acid (AA) levels were measured in the wound tissues by spectrophotometric method. Wound tissue was examined morphologically and histologically. MDA levels were significantly decreased in all treated groups compared to the untreated group (p< 0.05). GSH levels were significantly increased in all treated groups compared to the untreated group (p < 0.05). NOx levels were significantly decreased in all treated groups compared to the untreated group (p< 0.05). Wound tissue MMP-2 and MMP-9 levels of all treated groups decreased statistically compared to the untreated group (p<0.05). Wound tissue collagen levels in all treated groups were statistically increased compared to the untreated group (p< 0.05). PC levels of the treated groups were statistically decreased compared to the untreated group (p<0.05). AA levels were statistically significantly increased in all treated groups compared to the untreated group (p<0.05). In morphologic examinations, wound closure rates of all treated groups were higher than the untreated group (p<0,001). As a result of histological evaluations, re-epithelialization, epidermal thickness close to normal, presence of keratinization, granulation tissue and remodeling close to normal were observed in all treated groups. Inflammation and necrosis were milder in all treated groups compared to the untreated group. The combination of C.spinosa and glutamine showed an oxidative stress-reducing effect on wound healing. The combination of C.spinosa and glutamine positively affected wound healing in the diabetic wound model. The results of this study suggest that the use of C.spinosa and glutamine components together or separately may be a promising strategy to reduce oxidative damage and enhance wound repair.
